Intake and Eligibility
To start the process, the Individual or the Individual’s legal guardian must make contact to the FCBDD themselves. Our services are voluntary, and we need to know that the Individual is aware of the application, and the Individual is choosing our services. To proceed with the Intake process, the Individual or legal guardian must complete an application, provide probate court records of guardianship if applicable, proof of identification, and proof of the developmental disability / qualifying condition, such as, school records of the IEP and ETR, medical and psychological reports.
Eligibility determination occurs at ages 3, 6, 16, and if the Individual has a significant change in functioning.
A person may qualify for services when they have a developmental disability / qualifying condition that is:
Once the qualifying condition is determined, an Eligibility Specialist will meet with the person, face to face, to complete a functional evaluation called the Children’s Ohio Eligibility Determination Instrument (COEDI) or Ohio Eligibility Determination Instrument (OEDI). This instrument is a statewide evaluation tool used to assess the impact of the individual’s disability on his/her functional abilities and to assess their life skills.
Once the evaluation is completed, the Individual and guardian, if applicable, will be provided the results in writing, along with information on the right to appeal the county board’s decision.
Eligible for SSA services: An SSA will be assigned and will contact the Individual to schedule a time to meet.
Ineligible for SSA services: The evaluation results letter will include recommendations and community resources.

- Manifested prior to age 22
